Option #1 – Case Study to Consider

Ann and Michael have been married for 55 years. Ann is 80 years-old, and suffers from lung cancer and advanced Alzheimer’s disease. She currently resides in a nursing home, and often does not recognize Michael when he visits. Last night she was admitted to the hospital with difficulty breathing. Today, you are the nurse caring for Ann, and her physician is suggesting surgery to remove part of her lung to potentially slow the progression of her cancer. Michael is feeling unsure about this course of treatment, and asks for your advice and guidance.

How would you respond to Michael and serve as advocate for your patient?

As we evaluate the cases for discussion this week one question comes to mind: Who defines what is ethical? Our readings mentioned Fleishman who defines ethics as integrity. According to Fleishman integrity means having a genuine disposition to do the right and just thing in all circumstances, and to shape one’s action accordingly. It is impossible to have a code of conduct for every possible situation therefore, it is up to each one of us to depend on our own integrity to do the right things and to make sure there are no violation of civil liberties.

According to Dobel integrity is a balance among the three domains of personal moral commitments and capabilities, obligations to the office and political prudence. Integrity depends upon self conscious reflection, honesty and the self disciplined to resist temptation.

As you react to the articles think of your own integrity and your own personal moral commitment to do the right thing and compare that to the information presented in the articles.

There are different types of managed care plans. Two of the most common types are Health Maintenance Organizations (HMOs) and Preferred Provider Organizations (PPOs). Most others are hybrids of the two. The basic functions of governance and control in HMOs, PPOs, and other managed care plans are similar to those in any business except the responsibilities vary greatly from one plan to another.

Managed care programs use several functions to control costs: use of a provider network with discounts, prior approval of procedures, utilization management of treatment guidelines, utilization review, case management, peer review and second opinions, prospective, concurrent, and retrospective audits, and grievance procedures.

Then, reflect upon this experience in a paper. Include the following in your report:

  • Provide an overview of objectives of the task and reflect on your experiences.
  • Analyze the managed care systems, including strengths and weaknesses.
  • Based on your experiences, provide recommendations and insights for future application.
